Dr. Frankenbaum will trigger off of every abomination that dies at the same time as them, including in combat. However, it is possible for Dr. Frankenbaum and another abomination to die in the same batch but not trigger the damage effect. For example, if there is a Flamebreak Invoker on the field and Dr. Frankenbaum gets killed by a Lightning Spark, the Invoker trigger will end up in the same batch as the Lightning Spark damage. If the Invoker deals enough damage to kill another abomination, both Dr. Frankenbaum and the abomination will die in the same death check at the end of the batch. However, since Dr. Frankenbaum recieved damage first in the batch, it will also be the first to die in the death check. When the other abomination dies, Dr. Frankenbaum will not be around to trigger the damage effect. If the other abomination were killed by the Lightning Spark and Dr. Frankenbaum by the Invoker trigger, the other abomination's death would trigger Dr. Frankenbaum's ability.
